3. A copy of the passport (notarized or authenticated by the corresponding authority)
4. Criminal background check
5. Health certificate
6. Certified check for B/250.00 infavor of the National Treasury
7. Certified check for B/800.00 infavor of the National Migration Service
8. Declaration of Personal Background form
9. Public Registry certification proving the company's social reason, the applicant's position as director or officer, the legal representative or attorney-in-fact, the company's capital (minimum B/.160,000.00), and the type of shares (must be registered shares)
10. Certification from the Secretary or Treasurer of the company, proving the applicant's ownership of the shares issued in their favor and that they are fully paid and released (shares with a minimum value of B/.160,000.00). This certification must be notarized and must not be signed by the applicant themselves.
11. Copy of the relevant pages from the company's Share Register and the shares issued in favor of the applicant.
12. Certification from the company's authorized Public Accountant (CPA) proving the applicant's ownership of the shares and that they are fully paid and released (shares with a minimum value of B/.160,000.00), and detailing the company's activities.
13. Declaration by the applicant before a notary public regarding the amount of capital invested, the number of shares held, and the company's activities.
14. Payment receipt for advance payment of capital gains tax and copy of the shares purchase and sale contract(if the applicant acquired the shares from a third party of an existing company).
15. Copy of the company's income tax return with a paid and cleared status (in the assets section of the income tax return, the line for shares must show the payment of the total shares, which must be a minimum of B/.160,000.00 per applicant)
16. In the case of a newly formed company, a business plan and projected financial statements for the next two years.
